Help you understand your plan
You have your NDIS plan… now what? The first step is deciding on a Support Coordinator who you feel will best support your needs. This selected person can then be your dedicated ‘go-to’ contact to assist you on this journey.
Your Support Coordinator will provide initial support through explaining the details of your NDIS plan with you, including terminology and acronyms that can overwhelming and that are foreign to anyone new to their NDIS plan.
Part of this process includes listening to you and understanding your goals, things that make you happy and what they all mean to you.
Your Support Coordinator will guide you through the complexities of your NDIS plan so you have a clear understanding of what your plan entails and how they can work together with you to achieve your goals and make the most of your funding.
Maximise your plan opportunities
Now that you have a good understanding of your NDIS plan, the next step is to decide how you want to use the plan to its fullest potential to ensure it aligns with your personal goals and aspirations.
Your Support Coordinator has an intricate knowledge of the NDIS Price Guide and Support Catalogue which are the two main documents that you need to understand in order to make the most out of your plan.
In addition to this, your Support Coordinator will assist in identifying gaps in your supports and services, and will help you explore additional options that may enhance your opportunities.
Using a specially designed program, your Support Coordinator will give you some insight into what each of your supports will cost you and help you to understand what that means in the context of the impact on your funding.
At the end of this session you will have a solid understanding of your funding and what support you can afford for the duration of your plan, ensuring that you are getting the most benefit - without the risk of running out of funding toward the end of your plan.
Connect you with service providers
Now that you understand what you can do with your funding, your Support Coordinator will review your Service Agreements with your existing providers (where appropriate) to ensure that your needs are being met and your rights are being protected.
If you have decided that you need additional support, your Support Coordinator has access to a wide range of quality services that meet your individual needs and preferences, as well as the ability to source new ones when appropriate to support your desired outcomes.
Your Support Coordinator will facilitate connections and conversations with NDIS registered service providers who can deliver the supports outlined in your plan.
As a minimum we expect that Service Providers will have a current Police Check, NDIS workers screening check and NDIS orientation completion certificate. If they are providing professional services, we will also check their professional qualifications.
Assist with provider negotiations
Once you have decided on the Service Providers that you want to engage, your Support Coordinator will support you to negotiate an appropriate contract with them.
A Service Agreement will then be prepared on your behalf ensuring that your needs are met and your rights are being protected.
We will also conduct regular reviews of your NDIS plan to assess its progress and effectiveness and we are proactive in making adjustments as necessary, ensuring that your engaged serviced providers continue to support your evolving needs.
