You source and agree to provide services to agency managed NDIS participants
You use your own service agreement to engage
Service Agreement to include that you are brokering through Collabor8 Supports
Service Agreement to include Collabor8 Supports as part of the complaint management process
NDIA consent to authorise Collabor8 Supports to liaise with NDIA
You complete the intake form which will be sent to you when we are ready to start working together
Collabor8 Supports creates service booking in PRODA; or
For participants who are in PACE, you support participant to contact NDIS to register McGrath Family Trust as trustee for Collabor8 Supports to be listed as an approved provider and ask that a Request for Service is sent to Collabor8 Supports for acceptance

You provide services to NDIS registered participants
You send invoice for services charged at 80% of the NDIS price guide for service provided
Email to invoices@collabor8supports.com.au
If you are registered for GST include GST in the invoice
Collabor8 Supports submits a claim for payment from PRODA
Payment received from PRODA
Collabor8 Supports pays the invoice submitted by you

You source and agree to provide services to agency managed NDIS participants
You use your own service agreement to engage. The service agreement needs to include Collabor8 Supports as part of the escalation process
NDIS consent form needs to be completed giving Collabor8 Supports authority to liaise with NDIS
You complete this intake form
Collabor8 Supports creates service booking in PRODA, or
For participants on PACE the participant needs to advise NDIA that McGrath Family Trust as trustee for Collabor8 Supports (Provider #4050071609) as their preferred provider and request that a request for service is sent to us

NOTE: We process invoices on Mondays so if you want the invoice paid as quickly as possible you will need to have the invoice to us by Sunday evening. Invoices will be paid by Friday depending on when NDIA process payments.
