Risk Management Policy

Collabor8 Supports is committed to the effective identification and management of risks, which may arise during the general management of the business and in the delivery of support coordination services.

The Director is ultimately responsible for identifying and managing risks that impact the business.

All staff are inducted on, and are required to actively participate in, the ongoing identification and implementation of risk control measures.

Collabor8 Supports’ approach to effective approach to management of risks to the participant will be developed in consultation with participants.

Risk controls and failures will be evaluated as part of our drive to improve the quality and safety of our services.

Human Resource Policy

Collabor8 Supports is committed to recruiting people who have the competence and appropriate qualities to undertake their role.

All staff, including contractors have their qualifications, experience, National Police Check, and ID verified before they commence work.

All staff commit  to upholding the NDIS Code of Conduct and abide by the requirements set out under the NDIS Act and Collabor8 Supports’ policies and procedures.

NDIS Code of Conduct

The NDIS Code of Conduct promotes safe and ethical service delivery, by setting out expectations for both NDIS providers and workers.

Collabor8 Supports requires all staff, including contractors to read and agree to the following Code of Conduct:

• act with respect for individual rights to freedom of expression, self-determination, and decision-making in accordance with relevant laws and conventions 
• respect the privacy of people with disability
• provide supports and services in a safe and competent manner with care and skill
• act with integrity, honesty, and transparency
• promptly take steps to raise and act on concerns about matters that might have an impact on the quality and safety of supports provided to people with disability
• take all reasonable steps to prevent and respond to all forms of violence, exploitation, neglect, and abuse
• take all reasonable steps to prevent sexual misconduct

Privacy Policy

Collabor8 Supports values the privacy of every person and is committed to protecting the privacy and dignity of all participants.

We have implemented safeguards to ensure the safety and security of participants’ personal information in line with the Australian Privacy Principles of the Privacy Act 1988 (Cth) and the National Disability Insurance Scheme Act 2013 (Cth) (NDIS ACT).

Personal information is not shared with anyone unless required by law. Mobility Logistics will only collect information that is relevant to the needs of service delivery for the individual. Collabor8 Supports will always endeavour to obtain personal information directly from the individual concerned and/or their guardian/family.

In collecting personal information Collabor8 Supports will take reasonable steps to ensure the participant knows: why the information is being collected, what the information is being used for, how the information will be stored and where, who has access to the information and why.

How the individual and/or their guardian/family can access the information held, seek its correction, raise questions or concerns about how Collabor8 Supports is dealing with the individual’s personal information and how we deal with such a complaint.

Feedback and Complaints Policy

Collabor8 Supports is vigilant in exercising a duty of care and reporting incidents when occurs. 

All incidents are investigated and analysed to reduce the likelihood of a similar event occurring again and drive continual improvement.

We encourage people to raise concerns about any aspect of our service, or their experience with other providers.  We investigate all reports of near misses and incidents that have occurred.

We will support people through the incident handling process, linking to external agencies such as Advocacy services and the NDIS Quality & Safeguards Commission.

Collabor8 Supports staff are trained on effective incident handling processes and commit to abiding by the NDIS Code of Conduct and NDIS (Incident Management and Reportable Incidents) Rules 2018.

Collabor8 Supports is committed to providing people with accessible mechanisms to raise concerns and report incidents including a Feedback, Complaint & Incident Brochure with information on the NDIS Quality and Safeguards Commission complaints handling mechanism.
